We offer full turnkey service to all our providers, starting with verifying each patient's insurance before shipping the product. After the product is delivered and used, we issue an invoice to the provider with net payment terms from the date of shipment.
A 40% prompt payment discount is available when payment is made before the net term due date. Failure to pay within the net term will result in a 10% surcharge added to the original invoice amount.
To get started, we require the provider to complete two forms: a Business Associate Agreement (BAA) for HIPAA compliance and a New Account Setup Form. Links to these forms are provided below.
